If you’re a provider wanting to refer a patient for sleep services, the process is simple.
Just complete the secure form by clicking the button below. The information will be forwarded to Dr. Garcia, and she will contact the patient within 1-2 business days. Please note that Dr. Garcia does not offer emergency services.
What happens next?
After Dr. Garcia receives the referral, she will reach out to the patient to schedule a complimentary 15-minute phone call. During that call, Dr. Garcia and the patient will determine goodness of fit and schedule an intake appointment if the patient desires. At intake, Dr. Garcia requests a release of information to contact the referring provider for care coordination.
