If you’re a provider wanting to refer a patient to Dr. Garcia, just complete the secure form by clicking the button below. The information will be forwarded to Dr. Garcia, and she will contact the patient within 1-2 business days. Please note that Dr. Garcia does not offer emergency services.
What happens next?
After Dr. Garcia receives the referral, she will reach out to the patient to schedule a complimentary 15-minute phone call. During that call, Dr. Garcia and the patient will determine goodness of fit and schedule an intake appointment if the patient desires. At intake, Dr. Garcia requests a release of information to contact the referring provider for care coordination.
